In-office treatment provides you the comfort of privacy as compared to other hospital or community clinics and settings. Confidentiality is extremely important and any information discussed is considered to be privileged and protected.

On the initial appointment a thorough assessment is performed, also known as a Diagnostic Assessment in which important information is collected to better understand how to help you and provide you with the possible treatment options.

To find out how much your health insurance company will reimburse you, please consult your health insurance brochure, or call your health insurance company either before or after your first visit. Important information to obtain from them would be as follows:

1. 1.Your “out-of-network” benefits when seeing a psychiatrist.
   

2. 2. Amount reimbursed for the following codes which may apply to your case:
   

    90801- Initial Consultation

    90806- 45-50 min Individual Psychotherapy

    90807- 45-50 min Individual Psychotherapy

                    with medication management

    90862- Medication Management with minimal therapy

    90847- Family Psychotherapy

3. 3.Amount of your annual deductible.
   

4. 4.Number of certified visits you have per calendar year.

Full payment is expected at the time the service is provided.

Fees are independent of any reimbursement from your health insurance company. Dr. Lopez-Leon does not guaranty any reimbursement from your insurance company.

Payments may be made by personal cheque, money order, cash, Master Card or Visa.
